PRESCRIPTION WEIGHTLOSS PROGRAM

Phyllis Brown, CRNP, Nurse Practitioner (under the supervision of Sheldon Milner, MD) continues the evolution of anti-aging management with a personally supervised prescription weightloss program involving vitamin B-12, optimizing your metabolism and controlling the weight gain battle. Phyllis brings with her over 20 years of medical and plastic surgery experience having worked at Johns Hopkins Hospital and Shock Trauma. She provides over 12 years of prescription weight control management and pre-operative surgical evaluation.

THERAPEUTIC AND POST-OPERATIVE LYMPHATIC MASSAGE; PILATES

Amy Tullis, CPT is our newest addition to the anti-aging management and cosmetic surgery care model. Amy is a licensed physical therapist in both Maryland and Delaware with a specialty practice in myofascial release, postoperative care, and women's health. She uses advanced hands-on-care to relieve muscle and joint pain, decrease post-operative edema and stiffness, and correct spinal dysfunction / alignment of the neck and back. Pilaties classes are available.

COSMETIC SURGERY AND MEDICINE; Achieving the NON-FACELIFT FACELIFT WITHOUT SURGERY

We have achieved a new era in Cosmetic Surgery and Medicine!

Patients today need and require to look good all the time, yet have the demands to return to work quickly with little down time. At the same time, people do not want the invasive techniques, but want the effects of a facelift.

Technology today has brought about incredible knowledge that allows us to used non-invasive techniques to achieve these facelift like effects with little to no downtime. Our practice is unique in this understanding and has designed very advanced approaches and strategies that uses all of today's newest non-invasive techniques. By combining all the state of the art services we uniquely provide for our patients a synergy (synergistic effects) that are long lasting and dramatically effective. We do this by understanding how and why we age and plan our "attacks" based on the aging of our face on three dimensional planes.

Today individuals want to achieve a facelift like effect without undergoing the scarring of a full face and neck lift. We can achieve this today by combining 3 approaches at the same time to get a synergistic effect. Our goal is to reach the 3 levels of the skin that causes aging and reverse the aging process. The 3 layers of skin involve the top layer, the layer we see, the epidermis, the second layer is the dermis just under the skin where we have our fat layer and the third layer is the muscle layer.

We have evolved to a wonderful understanding that if we work on all three layers of skin at the same time we can achieve a facelift like effect without necessarily needing invasive surgery.

We start with the top level of the skin using both skin products that exfoliate and stimulate skin regeneration of collagen, elastin, hyaluronic acid and new skin cells. We use the Obagi NuDerm Skin line and the use of various exfoliating therapies such as chemical peels, laser and microdermabrasions. The regeneration of new skin is crucial to maintain a tight smooth glowing skin free of sundamage, pigmentation, fine lines and wrinkling.

Aging is a process where the skin is biologically slowing down as we chronically age. The skin loses its ability to regenerate itself at a fast rate (like children teens and 20 year olds). After 30 our skin begins to slow and the signs of aging is evident by seeing brown pigmentation, fine lines and wrinkles, red veins and hughs. The dead layers of skin build on top of each other and no longer exfoliate. The more the dead layers build, the more we see aging of the face. To reverse this process and to stimulate face to regenerate new skin cells they must be stimulated and exfoliated.

The advanced combination of the Obagi Nu Derm Home System with exfoliating and stimulating skin therapies is required to stimulate new beautiful tight smooth skin that will be needed for the rest of your life. Yes, the rest of your life, because you lack the ability to stimulate your own skin to regenerate. Aging means dependent on outside sources to keep us young and fortunately we have the products and the therapies to do it today without needing surgery!!!!

The second layer of the skin involves the fat thickness. As we age we tend lose fat, collagen, elastin, hyluranic acid and the important ingredients that plump our faces. When we lose this thickness we lose volume of the face. Deflating the face is like deflating a balloon as the balloon loses air it looks shriveled and shrunk. We see the same in the face, our lips loose fullness and lipstick lines in and around the mouth grow deeper, our lips look thinner and lack that plump pretty look. The facial skin around the cheek and jowl become evident, because we not only lost fill, but the elastin that holds up our skin tone is diminishing so deep nasal folds and jowls are seen. The eyes tend to look sunken, because as the cheeks fall they draw the eyes down with the entire skin laxity complex.

We have several solutions to reverse deflation and skin laxity. It can range from as simple as using appropriate fillers such as restylane, radiesse or juvederm and strategically refill the lost volume of the lips and nasal folds and marionette lines. A new concept being introduced is the volume lift that strategically puts fillers in the upper cheek area, under the eye hollowness that not only provides woman with a high cheek bone appearance, but causes the skin to redrape lifting the skin laxity away from the nasal folds. You get two affects at the same time, tightening and fill. When a filler is placed directly above the hair of the eyebrows, it lifts the eyebrows to a arched approach and pulls the hooding up and away off the upper eyelids. Again a two for one effect, a browlift and upper eyelid lift without surgery. Add Botox into the mix and you get a true non-surgical browlift.

This brings to the next or third level, the muscles of the face. Muscles in the face are like any muscles in the body, the more you use them and work them the stronger and thicker they become. Unfortunately, the face is not one place where we like our muscles to looked well exercised. It's the only part of our body that we want to weaken our muscles rather than strengthen them. The combined affect of muscle thickening with volume lost produces lines, wrinkles and folds where we don't want them. For example, the losts volume in our lips combined with the thickening of our lip muscles creates more prominent the lipstick lines. The musles of our forehead and between our eyebrows show angry, tired and heavy hooding of our brows and eyes. For both situations the combination of botox stop the muscles from "exercising" or from thickening and the fillers fill in where large creases and crevices have formed from over heavy use of those muscles.

When you take all three approaches at the same time, we achieve a NON-FACELIFT FACELIFT. Patients undergo a chemical peel, often a TCA or Obagi Blue Peel, Restylane in the lips, cheeks, nasal folds and Botox in the lips, and forehead. An overall tightening, lifting and filling synergestic affect brings about a much more youthful face.

COMBINING THE NON-INVASIVE APPROACHED WITH SEMI-INVASIVE APPROACHES

Unfortunately, some of us experience more laxity than a filler can lift the cheek and jowl area. When actual skin has to be lifted and redraped we now have the new CONTOUR THREADLIFT technique. This approach has gained so much popularity, because it only requires two tiny incisions in the hairline above the ear and the Contour Threads are strategically inserted into the face to elevate the skin of the cheeks and jowls. In our practice, Dr Dean Kane is one of the more advanced physicians in the United States doing an advanced technique that combines the use of facelift like technique while inserting the threads achieving a much greater tightening and lifting affect. The recovery is very quick and fairly painless.

We have found that Threadlifting is best used for lifting cheeks and jowls, while the short scar lifts are used for browlifts and neck lifts.

Applying our synergistic approach with the Contour ThreadLift Technique, we always encourage the Obagi Nu Derm skin stimulation system with an Obagi Blue Peel and Botox and Restylane. Again the synergy of all procedures done at the same time gives a giant net effect that is quite dramatic and long lasting.

SHORT SCAR COMBINATIONS WITH THREADLIFTING, BOTOX, RESTYLANE, CHEMICAL PEELING

Many of us want it all without the evidence of scars. The short scar procedures are often incorporated with the Contour threadlift, Botox, Restylane, Obagi and Chemical Peels.

The short scar procedures are those used on the face where we need surgical removal of excess skin because simply -there is just too much skin- and it needs to be removed to get adequate lifting of the face. What is unique about this concept is that we are removing moving skin, but relying on minimal of scar on the face. The scars are again strategically placed where they will not be seen. In the forehead, where Botox is not sufficient to adequately raise the brow and elevate the hooding of the upper eyelid, an inch to inch and half of scar is placed in the hairline to remove the excess skin and smooth the lines of the brow.

The Short Scar is a custom-like facelift in which we customize the length and placement of the scar for each of our patients trying to minimize the need for as little scar as possible. So often times we would use a short scar for a browlift, but use the threadlift for lifting the cheek and jowls. Another combination would be using the short scar neck suspension with Threadlift to the jowls. In this duo procedure the patients are seeking to achieve a tighter neck, chin and jaw line. Through very tiny incisions under the chin and behind the ears, lipo contour of the neck and jaw line is combined with tightening the muscles of the neck and then the Contour threads are used to lift the skin off the neck through the threading of the jowls.

Short scars to lift the skin of the cheek and jaw line are frequently used when a patient is not a candidate for a threadlift procedure. This is the case when a patient has just too much to be removed or a patient's skin and face is too thin where the threads even placed deep can be seen. Patient selection is extremely important for the right procedure and we evaluate the patients intensely to provide the right procedure for the right patient as well as the right patient for the appropriate procedure. This match couldn't be more important.
